Fine and Visual Motor Baseline Assessment is a data form that can be used to assess a student's ability to complete FUNCTIONAL skills such as connecting dots, following mazes, coloring, following a directed drawing, completing a word search, calibrating glue, cutting on a line, cutting simple shapes, cutting complex shapes, rotating a pencil to erase, coordinating both sides of their body, AND engaging a hole punch.
 
This tool corresponds seamlessly with the Monthly Progress Monitoring Forms and is ideal for initial assessment. This packet can be used by an occupational therapist, special education teacher, or elementary teacher to gain data and write goals.
