Pre-Writing and Handwriting Baseline Assessment is a data form that can be used to assess a student's ability to write the letters of the alphabet (uppercase and lowercase) and numbers 0-9, draw various pre-writing lines and shapes, write his/her name, copy a sentence from both a near- and far-point model, dictate a sentence, AND compose a sentence on their own.
 
This tool corresponds seamlessly with the Monthly Progress Monitoring Forms and is ideal for initial assessment. This packet can be used by an occupational therapist, special education teacher, or elementary teacher to gain data and write goals.
